Our online Request an Appointment form allows you to share your contact details, preferred dates and times, and the type of service you’re seeking whether it’s primary care, psychiatric evaluation, TMS consultation, or wellness services.
Once we receive your request, a member of our team will contact you to confirm the appointment, review any additional information we need, and answer any questions you may have. If you are a new patient, we may send you forms to complete before your visit to help save time on the day of your appointment.
If your concern is urgent, we provide clear instructions on how to reach our office directly by phone. For medical emergencies, we remind patients to call emergency services or visit the nearest emergency room.
